Commissioner Peter McInerney QC has handed down his final report from the Special Commission of Inquiry into the Waterfall Rail Accident.
The accident occurred on 31 January 2003 about 2km south of Waterfall station.
Commissioner McInerney's report details the causes of the accident and the factors which contributed to it.
